Blue Mountain Care Center, a 40-bed, intermediate care facility in Prairie City, is a nonprofit organization that is part of the Blue Mountain Hospital District. We are Medicaid certified only (not Medicare), providing long-term care, day care and respite care.

Our mission is to make each resident feel like part of our family. It is the goal of BMCC to keep each resident functioning at their highest level of independence while maintaining a high degree of dignity and respect centered around their personal wishes. Each resident must have a local physician who will make rounds at the care center every 30 days, for the first 90 days, then every 60 days thereafter. Appointments can be made at the physicians’ office more often if necessary; nurses are in contact with the doctors as necessary also. Blue Mountain Care Center employs licensed nursing staff 24 hours a day and certified nursing assistants to provide the day-to-day care for our residents. Blue Mountain Care Center has always met or exceeded the state’s mandated staffing ratio.

We have an excellent dietary staff that serves delicious home-cooked meals under the supervision of a registered dietitian. They offer special diets to meet every resident’s needs. The last Thursday of each month, the residents create their own menu for the lunch meal. Family and friends are encouraged to attend. Blue Mountain Care Center also has a full-time social service department that meets the psychosocial needs of each resident individually. The activities department provides a large range of activities such as gardening, picnics, fishing trips, weekly drives, outings to events and regular lunches at the senior centers.

Activities are centered around the preference of the resident, trying to keep them involved in activities just as they were prior to placement.

The care center has a bus that transports residents to and from local medical appointments, as well as all the out-of-facility activities. We provide basic haircare to residents as part of their all-inclusive rate through the activities department. A licensed beautician comes in monthly and as requested. Haircuts and manicures are included in the basic daily rate for the care center. We have housekeeping services provided five days a week and laundry services six days a week, also included in the all-inclusive rate. Visitors are welcome at the care center anytime. Come see our updated lobby, floors, paint and decorations. We are happy to give tours. Each room is painted in modern colors with an accent wall complimented by color-coordinated curtains. Private heating/air conditioning units are in individual rooms. Personalizing the room makes it feel more like home. Placing a family member in a care facility is a difficult decision.
